How to use it

Record it plainly, count it honestly, bring it with you

Step 1

Fill in the cycle numbers section from your own records the evening before.

Step 2

Mark the two or three symptoms that bother you most — an appointment rarely has room for ten.

Step 3

Write the answers down during the appointment; nobody remembers them afterwards.

Why write the numbers down beforehand?

Because "my cycles are a bit irregular" and "my cycles have run between 24 and 38 days over the last year" lead to very different conversations. Specific numbers turn a vague complaint into something a clinician can work with.

What should I not bother listing?

Everything you noticed once. A symptom is worth raising when it recurs — the checklist asks for how often, precisely so a single bad day does not crowd out a real pattern.
